Eligibility
If you are a full-time associate of Four Seasons Family of Companies, regularly working over thirty (30) hours per week, you are eligible to participate in the Healthcare and Benefits Program on your 30th day of employment. You have the option to enroll yourself and your eligible dependents in Four Seasons' benefits program.

If you do not enroll within 30 days of your initial eligibility period, you will not have an opportunity to do so until
the next open enrollment period, unless you have a Qualifying Life Event, such as:

Marriage or divorce

Birth, adoption, or legal guardianship

Change in work status that affects your benefits

Change in a dependent's eligibility of benefits

Death

Eligible dependents include:

Your legal spouse*

Your children up to age 26

Unmarried disabled dependent children of any age who depend on you for support

*Legal spouses are eligible for medical and prescription drug coverage offered by the Four Seasons Family of Companies, provided that your spouse is not employed OR your spouse’s employer does not offer medical insurance. Spouse eligibility only applies to medical and prescription drug coverage. You can enroll your spouse in dental and vision coverage regardless of employment or another coverage status. If you enroll your spouse in a Four Seasons Family of Companies medical plan, you are required to complete a Spousal Coverage Verification Form. The complete form must be returned to the Human Resource department.
